To optimize the distribution of health information to support patients, health plans must process thousands of data files on a regular basis, ranging from provider and pharmacy claims to lab tests and hospital bills. When properly managed, this data can help identify which health plan members are most at risk for complications from diabetes or pregnancy, or one of many other complex medical issues.
Now, Hudson Health Plan (Hudson), a Medicaid managed care organization based in New York, has found a solution to automatically mine the mountain of available data to make sure members who need extra attention are put into the capable hands of its nurse case managers, as well as better manage the care of all its members.
Hudson is implementing a comprehensive and integrated care management software system offered by TCS Healthcare Technologies (TCS) called Acuity Advanced Care™ (ACUITY). The ACUITY system is a best-in-class IT platform that will support Hudson’s Utilization Management (UM), Case Management (CM), Disease Management (DM), and Prevention/Wellness (PW) services.
